Consent Form
I hereby give my consent for the following:
I acknowledge that Indian Health Organisation Private Limited (MediBuddy vHealth/IHO) has informed me of a change from the originally purchased preventive 31 blood tests package to an alternative point of care 31 preventive health tests package.
I understand that the alternative package is performed in place of the original package for which I previously provided consent.
I confirm that I have no objection to taking this alternative test package and agree to this change voluntarily, without any pressure or coercion.
I am fully aware that the alternative test is a type of point-of-care test, which may include, but is not limited to, Body Vitals, Body Composition Analysis, Diabetic Profile, or Iron Profile. I understand that this differs from my original test, which was a blood sample collection test including, but not limited to, Lipid Profile, Thyroid Profile, Liver Profile, or Renal Profile.
I understand that the alternative package is performed in place of the original package for which I previously provided consent.
